Hours:  Full-time (Monday-Friday, 8:30am-5:00pm)   PRIMARY PURPOSE Nurse Practitioners deliver primary clinical care per the PCMH/NCQA model of care for patients of Lorain County Health & Dentistry in the clinical office setting. Nurse Practitioners are responsible for maintaining and being compliant with a written standard of care arrangement with all collaborating physicians.
